After receiving vital services and emotional support during their stay, many families encounter obstacles when they leave. Our Community Resources Specialist is here to assist families in accessing essential resources for a smoother transition. Through our House-to-Home Case Management program, we ensure every family has the tools needed for optimal health outcomes for their children.
